Our Programs

IMAG0100RISE Life Services currently runs and operates the below programs. Each program is near and dear to our heart and is executed to the highest standards. We feel that with our excellent staff and program structure, we can make a difference in our residents lives. Please browse through each of our programs below to get a better idea of what we do and what we have to offer our community.

ICF (Intermediate Care Facilities) Program

Our ICF programs provide 24-hour supervised care to individuals who require enhanced supports and services.  Individuals residing in this setting are unable to care for their own basic needs including adaptive, medical and/or behavioral needs and require structured environments to assist them in reaching their full potential.  This setting is community-based and fosters a home-like environment to all of the residents.  Eligibility through the New York State Office for People with Developmental Disabilities (OPWDD) is a requirement of this program. IRA (Individualized Residential Alternatives) Program

Our IRA programs provide individuals with a safe home-like environment that encourages individualized service options.  This program can provide both 24-hour supervision or provide staffing to assist individuals with need-based supports.  The hallmarks of this program are highly individualized, person-centered supports and services. Participants of this program have choice in their habilitation services and goal areas and are the driving force of their life plans.  Eligibility through the New York State Office for People with Developmental Disabilities (OPWDD) is a requirement of this program. Read more…

MH (Mental Health) Housing Program

Our Mental Health Housing Program consists of three levels of housing with varying levels of supports and services.  Our Community Residences provide 24-hour supervision to individuals working towards moving into a less restrictive environment.  Our Apartment Treatment programs offer individuals need-based supports and our Supportive Housing Programs offer individuals an landlord-tenant relationship with minimal staff support.  All of these levels of housing offer individuals with eligibility in the New York State Office of Mental Health a therapeutic environment to increase their independence and inclusion in their community. The Main Street Connection Day Habilitation Program without Walls

Participants of this innovative program are afforded the opportunity to use their current skill and build upon their skill set to integrate into the community.  The day program without walls is a program offering choice of day activity with more flexibility than a traditional certified site-based program.  Social, academic and life skills training are the centerpiece at Main Street Connection Day Program. Northville Sensory Gardens

This 2.5 acre property boasts the most pristine sensory garden experience on the East End of Long Island.  The Sensory Gardens are the home to chickens, ducks, goats, our mini horse Cassidy and our potbelly pig, Meatball!  Many of our program participants work on the property to ensure its upkeep and seasonal plantings take place.  Our Sensory Garden Committee made up of board members, local businesses and farmers, community groups, and interested neighbors are working to enhance the use of the gardens and ensure that all can enjoy this beautiful property. Food Pantry

The food pantry is open every Wednesday from 9:30 am to 2 pm and is located at our main office at 901 East Main Street, Riverhead, NY 11901 Suite 500.  The food pantry is open to all community members and is used to supplement the supply of food to those in need.  Our food pantry receives donations from local businesses, and Island Harvest.   We are so proud to be able to help our local community with this much needed service.
